A Case of Eosinophilic Leukemia

Abstract
Eosinophilic leukemia is a rare disease and there has been much controversy regarding the classification of eosinophilic leukemia. However, Bently proposed specific criteria to establish this diagnosis ; "Marked eosinophilia in peripheral blood with blasts above normal in the bone marrow and/or peripheral blood during the course of the illness". Only one case has been reported in Korea. A 44 year-old female was seen on February, 1987 in the Kangnam St. Mary¢¥s Hospital Internal Medicine Clinic for evaluation of dyspnea and chest discomfort. Examination revealed a marked hepatosplenomegaly and there were signs associated with congestive heart failure. The initial white blood cell count was 217,200/mm3, with 78% eosinophilic series and 2% myeloblasts. First bone marrow examination revealed about 100% cellularity, about 61% were of eosinophilic series at all stages of maturation and there were 5% myeloblasts. Second bone marrow examination was performed 15 days later, but considerable improvement was not noted. Skin and liver biopsies revealed diffuse eosinophilic infiltration, the majority of which were of mature eosinophils. We believe, therefore, this case represents a case of eosinophilic leukemia. A brief review of the literature and a discussion on the clini¡þcopathological correlations were made.
